Engaging Activities for All

All of our residents will get a chance to participate in the same events and activities, although with different levels of engagement based on their cognitive ability and the stage of their disease. For example, residents in our early-onset memory loss living hall will have a more hands-on event experience than residents in our severe memory care living hall, who will attend their own version of the event with more staff support.

Germantown Lifestyle Journal

 As part of our personalized care plan, our staff and you will sit down during our initial visit and begin work on the lifestyle journal. We know how monumental this move in life can be, so we want to ensure our staff gets to know more about the resident’s history and their potential behaviors. We ask the family to help complete the lifestyle journal by move-in day. After the completion of the journal, we will review our care plan with you so that you have a say in how your loved one is treated while in our care.

Lifestyle journals are the centerpiece of our memory care social model. Each journal is filled with major events and memories from the resident’s life, such as a wedding day, past job, military experience, etc. We use this journal to get to know your loved one and have a catalog of their experiences to help monitor their cognitive progression while living in Germantown.

Progressive Memory Care for Every Stage of Dementia

Each resident will be placed in a specific hall tailored to the level of their cognitive ability and the stage of their disease. Each hall has its own dedicated staff, allowing each resident to develop personal relationships with their care partners. Each staff member is dedicated to getting to know and understand each resident and is highly trained to track the progression of their mental condition.

We use several assessments to monitor each resident’s progression, but we never lose sight of the real person. We use the lifestyle journal to connect with the resident and see how their disorder has affected their recollection ability based on the memories put in the journal. This means our staff can quickly determine when it is time for a resident to move to a new building based on the progression of their Alzheimer’s, organic brain syndrome, or other memory disorder.

Beale Hall: Early-Onset Memory Loss Living

In our early-onset memory loss community, your loved one can socialize, interact, and spend time with other seniors in the hall. Typically, these residents have fewer sensory issues than residents in our other two halls, which enables events in Beal to be more hands-on. Beal residents can handle more sensory functions; you’ll often hear music, conversations, and laughter from this hall. Our cognitive support assisted living is perfect for residents who can still function independently but need more support than an independent living community offers.

Poplar Hall: Mild Cognitive Impairment Community

At Poplar, your loved one will receive personalized care designed for their changing needs. Residents will enjoy tailored community events that manage sensory awareness, keeping them engaged, and promoting a sense of peace and joy. Because residents at Poplar typically need more help performing daily functions like getting out of bed, our caring staff will be there to help them while prioritizing their sense of dignity and comfort.

Union Hall: Severe Memory Care Community

Union Hall prioritizes making the last stages of dementia as meaningful as possible. Here, care teams work one-on-one with your loved one to ensure their comfort, health, and safety needs are always met.
